Bring a large pot of salted water to a boil for the pasta.
~8 minWarm the olive oil in a wide pan over medium heat and cook the finely chopped onion gently until soft and golden, about 4 minutes.
~4 minAdd the crumbled sausage, breaking it into small pieces with a fork, and brown it for 5 to 6 minutes until no longer pink.
~6 minPour in the white wine if using and let it cook off for a minute, then lower the heat and stir in the cream. Season with a little salt, black pepper and a grating of nutmeg, and simmer gently for 3 to 4 minutes until slightly thickened.
~4 minCook the conchiglie in the boiling water until al dente, then drain, reserving a cup of the pasta water.
~10 minAdd the drained shells to the pan with the sauce and toss over low heat, loosening with a splash of the reserved pasta water so the sauce coats the shells.
~2 minTurn off the heat, stir through the grated parmigiano reggiano until creamy, and serve hot with extra cheese and black pepper.
